Passports Services

The City of Upland Passport Office is located at the Gibson Senior Center, 250 N. Third Avenue, Upland.  

Passport Office hours of operation:

  • Monday through Friday, 11:30 a.m.-4:30 p.m.
  • Saturdays, 8:00 a.m. to 1:00 p.m.

As a Passport Acceptance Facility, we can assist you with submitting your application to the Department of State.  Renewal applications, Form DS-82, cannot be processed at the City of Upland Passport Office.  Routine service can take up to 11-weeks from the day an application is submitted at an acceptance facility to the day a new passport is received by the customer.  

An expedited passport is available for an additional $60 fee. Expedited service can take up to 8-weeks from the day an application is submitted at an acceptance facility to the day a new passport is received by the customer.  The time frame includes time for processing and up to 2 to 3 additional weeks for mailing time on the front and back end.  

Applicants for Adults must be able to provide the following for a new passport:  

  • A completed Passport Application, Form DS-11. Do not use white out, scribble, or cross out. 
  • Proof of U.S. Citizenship.  Ex:  original Birth Certificate, Certificate of Naturalization, or other proof of Citizenship  
  • A black and white copies of all required documents. 
  • Proof of identity Ex: Driver's license, State I.D. card (California) If out of State Identity must bring in other form of Identification Ex: Costco card, Credit card 
  •  A black and white copy of Proof of Identification front and back. 
  • One copy of a recent color passport photograph.  Many locations are available for this service, or the photograph may be taken at the City of Upland Passport Office for a $12 fee.

  • Children, ages 15 years and under must have both parents appear or the appearing parent must have a signed notarized consent form (Form DS-3053) from the non-appearing parent. See additional details below.   

Applicants for Minor must be able to provide the following for a new or renewing passport:

  • A completed Passport Application, DS11
  • Proof of U.S. Citizenship.  Ex: original Birth Certificate, or other proof of Citizenship  
  • A black and white copy of the Proof of U.S. Citizenship. 
  • Proof of identity for both parents Ex: driver's license, State I.D. card (California) If out of State Identity must bring in other form of Identification Ex: Costco card, Credit card. If both parents cannot attend the appointment, a completed notarized Form DS-3053 from parent who is not present along with copy of I.D. back and front are required. If more than one minor is getting a passport a completed notarized Form DS-3053 is need for each minor’s application 
  •  A black and white copy of all required documents, including proof of identification front and back for both parents. If more than one minor is getting a passport a copy is need for each minor’s application  
  • One copy of a recent color passport photograph.  Many locations are available for this service, or the photograph may be taken at the City of Upland Passport Office for a $12 fee.

RENEWALS:

  • A minor's passport may not be renewed by mail. To renew a minor's passport, you need to bring the expired passport and/or card, a copy of their birth certificate, a completed notarized Form DS-3053 (if applicable), a new, first-time passport application.  

  • An adult passport may be renewed by mail if it meets certain requirements. 

Check Your Eligibility to Renew by Mail (adult passports only)

  • Is the passport in your possession to submit with your application?
  • Is the passport undamaged other than normal "wear and tear? 
  • Was the passport issued when you were age 16 or older? 
  • Was the passport issued within the last 15 years? 
  • Was the passport issued in your current name or you can document your name change? 

If you answered 'Yes' to all 5 eligibility questions in the section above, you must mail your renewal form directly to the State Department.   If you don't meet all the renewal requirements, you'll need to schedule an in-person appointment, complete a new, first-time passport application, and bring all required documentation. 

DEPARTMENT OF STATE FEES: Only Check or Money Order is accepted for fees made payable to the U.S. Department of State for passport fees listed below. Each application/application needs an individual check or money order. 

  • Passport Book Adult (Age 16 & over) $130 
  • Child (Age 15 & under) $100
  • Passport Card Adult (Age 16 & over) $30
  • Child (Age 15 & under) $15
  • Book & Card Adult (Age 16 & over) $160
  • Child (Age 15 & under) $115
  • Expedite Fee Adults & Children $60 Additional Fee 
  • Overnight Postage Adults & Children $19.53 Additional Fee 

CITY OF UPLAND FEES:  Check Money Order, Cash, or Credit card (not Discover) are accepted for fees made payable to the City of Upland. Fees listed below are per application, but final payment may be combined for fees payable to the City of Upland.  

  • Acceptance Fee (Required) Adults & Children $35
  • Photos - 2 Prints (Optional) Adults & Children $12 
  • Copy Service - Flat Rate (Optional) Adults & Children $3 
  • Overnight Postage (Optional) Adults & Children $28.95
